I think it is real. As I understand women sometimes feel the need to pee during an orgasm (my gf told me) and of course they don't give in to that urge, because you'd need to clean everything up and guys don't usually appreciate being peed on ;) But if they do give in to that peeing sensation, they'd squirt. As far as I know women can't actually pee while being arroused, just as the other 'Anoniem' said earlier.
